Usually the implant is replaced with a similar one or the patient may wish to make some changes. Patients can choose a new size, shape or type of breast implant or may opt not to have a replacement at all.

If no replacement is chosen, or if there has been notable weight loss since the previous implant surgery, it may be preferred to have a breast lift surgery. This can happen at the same time as the implant removal and can greatly enhance the effect of the final outcome.

In breast augmentation surgery, the body forms a thin layer of scar tissue around the implant, called a capsule. capsular contracture (often mistakenly referred to as encapsulation), is when the scar tissue becomes thick, causing hardening or distortion of the breast implant, resulting in a change in its natural appearance. Infrequently there may be some tenderness or pain. The formation of a scar tissue capsule around a foreign object in the body is a perfectly normal immune system response and is nothing to worry about unless symptoms cause discomfort or distortion. In such cases it is best to remove the implant and surrounding scar tissue to alleviate the discomfort. This type of breast implant revision is known as a Capsulectomy.

En Bloc Capsulectomy is simply when the implant and scar tissue are removed altogether as one piece. This is typically required if the implant has ruptured, the capsule has developed calcifications, or for patients who want to ensure that the implant and capsule have been removed in their entirety. Breast Implant Removal Procedure – What does it Involve?

The breast implant removal procedure itself can be simple and quick, or may take more time if an en bloc capsulectomy is recommended. In most cases the implant can be removed through the same route it was inserted. The incision is made along the existing scar to limit any unnecessary added tissue damage to the skin. If a silicone breast implant was used it can be deflated before removal and a much smaller incision made. In the case of ruptured silicone breast implant removal, or en bloc Capsulectomy, a larger incision is required. Either way the incision will usually be in the same location as the existing scar and little to no new scarring will be added.

At this time the patient may desire to include breast lift surgery to remove any sagging skin that may have occurred since the augmentation procedure. A breast lift can also offset any stretch in the skin that occurred to accommodate the new implant. If no replacement implant is being added, then some degree of breast lift may be advised. One can expect bruising, swelling and some pain at the site, and we recommend refraining from strenuous activity for 6 weeks.
